This section is a “one-stop-shop” for everything you need to apply for an individual Dare to Dream award – from your application forms to templates for final reports once your project is completed.
To complete your application and submit it to the Centre, just follow the 10 easy steps listed below.
Dare to Dream applications must be submitted to the Centre before 4:00pm on October 31st 2011.
Who counts as an individual applicant?
If you are applying for an award and planning to do a project by yourself – supervised by your mentor – you should apply as an individual applicant. If you are doing your project with a group of youth, you should apply as a group applicant. Individual applicants are eligible for up to $1000 to help bring their mental health awareness projects to life.
Individual applicants may request an honorarium of up to $200 from that $1000 as a "thank you" for all their hard work and commitment, or, if they prefer, they may use the full amount of $1000 to fund their project. (Applicants are asked to indicate whether or not they would like an honorarium in the Project Budget section of the Dare to Dream Application Form.) You will receive your honorarium after we receive your final report.
What do individual awards fund?
- Supplies needed to bring a project to life (i.e. art supplies, production costs, food at events, prizes, equipment rentals, printing costs for promotional materials etc.).
- Honoraria for guest speakers at events (if they are not donating their time).
- The cost of training that is related to the project.
What do individual awards NOT fund?
- Capital expenses (i.e. heating, rent, hydro, phones, photocopiers etc.).
- Administration costs.
- Any budget items that appear to be inflated.

1. Read the Criteria and Conditions and General Guidelines before filling out the Application Form.
2. Make sure that you have a mentor who is experienced in child and youth mental health. (For more information on mentors, visit the mentor information section.)
3. Print out the application form.
4. When you have completed the applicant section of the application form, have your mentor complete the mentor section and sign where signatures are required.
5. Ask your mentor to review the entire application form.
6. Check carefully to make sure that you have filled in all required information.
7. Complete the Application Checklist (available on the last page of the form).
8. Contact the Centre (D2D@cheo.on.ca) if you have questions or are having difficulties completing the form.
9. Mail or hand-deliver your application package before the deadline. Please be sure to leave enough time for the mail system to deliver the application. We suggest mailing the application at least 1 week before the deadline.

Successful applicants will be notified approximately five weeks after each deadline, at the beginning of May (for the spring competition) and December (for the fall competition).
A word about your final report…
Once you have completed your project, you must complete a Final Report and submit it to us one month after the end of your award term. The Final Report Template provides six questions to guide you through this process.
In addition to the Final Report, an Accounting Summary must also be submitted. This document is to be filled in by your mentor.
It is a good idea to download and print these forms for your mentor as soon as the funds have been awarded at the beginning of the award term.
When sending in your final report you need to send in objects and/or visuals relating to your project. While doing your project make sure to collect pictures, t-shirts, posters, etc. so that we can showcase your project on our site!
If you have requested an honorarium, these are sent out once we receive your project’s Final Report.
